  • Form 1095-A if you enrolled in an insured plan through the Marketplace (Exchange)
  • Marketplace exemption certificate if you applied for and received an exemption from the Marketplace (Exchange)

Income

  • A copy of your previous years tax return
  • W-2 forms from ALL 2017 employers
  • Any unemployment compensation
  • Any alimony
  • Any winnings from gambling like the lottery, race track, casinos, raffles, etc.
  • Any miscellaneous income like prizes, awards, jury duty, etc.
  • Did you receive a state tax refund
  • Any pension, annuity, ROTH, IRA or retirement income
  • Any interest on savings, cash, US bonds, stock dividends
  • Any stock, mutual fund, or other securities sold
  • Any 1099-MISC
  • Did you own a business or were self-employed
  • Did you own rental property
  • Any royalties
  • Did you operate a farm
  • Did you sell your home or any other property
  • Did you receive installment payments on property sold
  • Any interest in a partnership, S-corporation, estate or trust
  • Any income as a minister


Expenses

  • Did you purchase a home
  • Did you pay alimony? (Name and SSN of recipient is required)
  • Any daycare expenses (Name, address and SSN (or EIN) of provider is required)
  • Did you have any college tuition expenses
  • Any interest on student loans
  • Any Home Mortgage Interest
  • Did you pay property tax on your home
